Medicaid & Public Assistance Programs

You may be able to obtain financial assistance through one of several public assistance programs. To apply for these programs, click on the links below to learn about each program and its requirements, and to pre-register. Then, print and complete the application and submit it to Valley Medical Center’s Financial Advocates.

Please note:

  • Many of these programs are not approved for retroactive services. Eligibility for these programs may provide coverage only for future services.
  • If you do not qualify for Medicaid or any other public assistance program, but are unable to pay for all of your care, we will ask you for documentation about your financial ability to pay to determine whether you may be eligible for assistance based on federal poverty guidelines.
  • You are responsible to know your Medicare benefits and non-covered services. You will be financially responsible for any non-covered services. Review our Notice to Medicare Beneficiaries.
