On April 22 we had a pipe elbow failure in our Emergency Department (ED) causing a significant water leak. Crews are assessing the impact and we are in the process of water abatement. Our ED is open, but several rooms are currently offline which could increase wait times. Some surgeries are also being rescheduled.
What services does the Senior Care Clinic provide to patients?
Senior care clinic is a primary care clinic with providers trained to meet the needs of seniors and issues pertaining to aging. We also do consultations regarding geriatric issues, with the primary focus on memory loss.
What are the target patient demographics and minimum age at this clinic?
Minimum age to be seen is 70 years old, but we can see younger if they meet criteria for geriatric syndromes on a case-to-case basis.
Does this clinic accept referrals for consultations for geriatric patients with complex conditions?
We do accept referrals for consultations via Epic for VMC providers, or via fax for affiliate or community providers at 425.690.9089.
When do you recommend a transfer of care to the Senior Care Clinic from another primary care clinic?
We recommend transfer of care for any patients over the age of 70 with complex medical conditions who want to focus on quality of life issues, function and mobility, medication management, and geriatric syndromes (such as memory loss/dementia, malnutrition/weight loss, frailty, and balance disorders).
