Lost Your COVID-19 Vaccination Card?

If you misplaced your immunization card that you received when you got your COVID-19 vaccinations (at Valley or at another site), you can download your record from the Washington State Department of Health (DOH) online system, MyIR. Unfortunately we are not able to provide replacement cards if you received your vaccines at Valley Medical Center.

- Create an account with MyIR for yourself (or your whole family)
- Print out the immunization card
