The POLST document can be a very nuanced and confusing document. WSMA in collaboration with Honoring Choices PNW has put together a toolkit found here. In this toolkit you will find information like a POLST Completion Guide for Clinicians and FAQs. We encourage you to review the Toolkit for your own edification and to help understand the appropriate use of the POLST document and to learn more about completing POLST with patients.
