The Desimone Levee on the Green River failed and is under repair. The hospital is above the flood zone and currently in no danger of flooding. We evacuated our Time Square and Kent Station Clinics and our Lind Avenue locations. Several locations are experiencing staffing issues due to severe traffic impacts. Impacted patients are being contacted to reschedule appointments. Please be safe, do not drive or walk through standing water, and call 9-1-1 if you need emergency evacuation assistance.

We have revised the ambulatory Advance Care Planning workflow to include the ambulatory providers' role in updating code status consistently (CLICK HERE for updated workflow) to see the full version of the educational tool that clarifies this process.
For appropriate patients, we encourage ambulatory providers to have meaningful goals of care conversations, update the code status accordingly and send the patient home with a congruent POLST document so they can carry their wishes outside of VMC.

The POLST document can be a bit confusing to understand. WSMA, in collaboration with Honoring Choices PNW, has put together a toolkit found HERE. In this toolkit you will find information like a POLST Completion Guide for Clinicians and FAQs. We encourage you to review the Toolkit to help understand the appropriate use of the POLST document and to learn more about completing POLST with patients.
