MyChart Diminished Capacity Access for Teens

What’s Happening?

Parents/Guardians of teens with diminished capacity can keep full MyChart Proxy access until the teens 18th birthday.

Who Does this Impact?

Clinic Providers, clinic support staff

Overview

  • Proxy access for teen charts changes for parents/guardians when the patient turns 13.
  • If the patient has diminished capacity, full access is suggested for continuity of care.
  • The parent/guardian can request a letter be sent from their VMC provider requesting this updated access [template name: Diminished Capacity Teen Attestation VMG]
  • HIM will receive the request and update the chart; route chart to HIM Leadership Pool
  • Legal documentation will be required to grant proxy access for adults with diminished capacity
