Parents/Guardians of teens with diminished capacity can keep full MyChart Proxy access until the teens 18th birthday.

Overview
|
- Proxy access for teen charts changes for parents/guardians when the patient turns 13.
- If the patient has diminished capacity, full access is suggested for continuity of care.
- The parent/guardian can request a letter be sent from their VMC provider requesting this updated access [template name: Diminished Capacity Teen Attestation VMG]
- HIM will receive the request and update the chart; route chart to HIM Leadership Pool
- Legal documentation will be required to grant proxy access for adults with diminished capacity
