Access Restrictions to Peri-Operative Areas

As of July 1, our new access restrictions are in place for all peri-operative areas. We will continue to adjust our workflows around access to peri-operative spaces on the hospital's 2nd and 3rd floors, to keep those areas secure and make sure procedures run smoothly.

We now have a new process to request access to these areas, including ORs, PACUs, Admit/Discharge, Endoscopy, and other associated areas. Staff or their supervisors can fill out this form, which will be located on the new Account Provisioning page of MyValley. Access is based on the employee’s job requirements and ability to complete their essential job functions.

Please use this process for new staff and transfers, who need badge access to Peri-Operative areas granted for their job duties.  Existing staff do not need a request filled out (unless access has been removed in error).  Once your request has been submitted, our team will review within 48 business hours.  If approved, your request will move to security, and you will be notified.
