Now Available: Direct Referrals to HealthyCare Sessions Through Epic

We’re excited to announce that providers can now refer patients directly to HealthyCare — our free, community-based lifestyle education session.

What is HealthyCare?
HealthyCare is a live session that helps breakdown perceived barriers, and provides education around many the challenges that we deal with when facing chronic disease as a community. It’s designed to spark motivation, provide guidance, and connect patients to the next step in their care journey.

Why refer?
This program is ideal for patients who:

• Need support making lifestyle changes
• Are managing chronic conditions like diabetes, high blood pressure, obesity, or heart disease
• Are not yet ready for clinical programs, but need a starting point

How to refer:
You can now refer directly through Epic by selecting “HealthyCare” in the referrals section, or by including this registration link in the After Visit Summary so patients can self register:

A SmartPhrase is available by searching in SmartPhrase Manager for HealthyCare.
