Diabetes Education Program

This program is designed to help you personalize your goals and give you the skills you need to best care for your health when you’re living with diabetes. Classes are offered in person and online, meet once a week for three consecutive weeks, and are taught by Certified Diabetes Care and Education Specialists (CDCES) who are also Registered Dietitians. Our Classes

Class 1: This class is focused on understanding what type 2 diabetes is, learning how to identify and prevent diabetes complications, and talk about how exercise affects diabetes. During this class you will also learn about diabetes medications, blood sugar monitoring, and get an introduction to exercise resources available at Valley’s Lifestyle Medicine centers.

Class 2: During the second class, we teach you how to plan diabetes friendly meals with the foods you like. This class is all about food and will answer all your questions about carbs, sodium, fats and more.

Class 3: The final class is designed to give you practice with real life situations you may face living with type 2 diabetes. It is a very interactive class and full of great tricks and ideas to help you feel confident moving forward.

Our diabetes program is accredited through the Association of Diabetes Care & Education Specialists and is well trusted by the providers in and out of Valley’s network. We provide evidence-based education and recommendations, giving you the opportunity to get trusted answers to your diabetes questions.

On average, patients have lowered their A1C levels by 1.4% within three months of completing our program. We also consistently see improvements in weight and cholesterol levels. Most importantly, patients report that they feel much more confident in their understanding of diabetes and their ability to manage this condition.

Cost

Nutritional Assessment: This includes a comprehensive evaluation of your dietary habits, medical history, and lifestyle to identify imbalances, or areas for improvement.

Management of Health Conditions: We offer specialized nutrition counseling for individuals with conditions such as diabetes, prediabetes, high blood pressure, high cholesterol, polycystic ovary syndrome, and more! This includes collaborative management plans to support overall health and well-being.

Weight Management: Providing evidence-based strategies for weight loss or gain, emphasizing sustainable and realistic approaches, and giving on-going support and monitoring to help you manage your weight.

Behavioral Counseling: Support you in identifying and addressing the behavioral factors that influence your dietary choices, and providing strategies to establish and maintain healthy eating habits.

Medical Nutrition Therapy is typically covered by most insurance plans. Valley Medical Center will bill your insurance company directly for these clinical services.
