Frequently Asked Questions

Provider Link is a Web-based version of the VMC electronic medical record system. Upon obtaining an account, you will have view-only access to the records of your patients who have been seen at VMC and VMC’s Clinic Network.

Complete the online new user access request form from the Provider Link Web site.Once submitted, the request will be reviewed and processed. Within 2-5 business days, you will receive your account details along with login instructions.

If you do not know your current password, please contact Customer Support at (425) 690-6200. A ticket will be opened for you and someone will respond within 1-2 business days.

Access to patients’ charts is automatically granted based on the provider-patient relationship. Providers listed as either the patient’s PCP or referring provider will gain access based on an existing recognized relationship.

Manual process

Submit an access request to - http://www.valleymed.org/providerlink.  In the request, comment field, add the following information of the patients you are requesting to have attached to your account:

  • Patient’s Name
  • DOB
  • Gender
  • Valley Medical Center MRN# (if known)
  • Reason for access
  • Note: Access to anything other than continuing care requires a valid authorization for release of information.

Each of your staff members will need to individually apply for access at:

http://www.valleymed.org/providerlink

During the enrollment process, staff members will be asked to provide the license numbers for all providers supported. We will verify the provided data and grant access to their patients.

While VMC encourages our Provider Link community providers to maintain their accounts, we understand circumstances including office relocation or retirement may warrant account closure.

Please send an e-mail request to donna_omeara@valleymed.org asking that your account be inactivated.

No. Once access has been granted, your account will remain open provided a minimal level of activity is maintained. VMC will perform an audit every six months. Accounts with no activity in the last six months will be inactivated. However, the user will be contacted prior to inactivation.

Users will receive a notification upon trying to log in stating whether the downtime is scheduled or unscheduled. Scheduled downtown notifications will include the date and start and end times. Otherwise, our technical team will be working to bring Provider Link back online promptly. We thank you for your patience.

Please direct all requests for medical records to Health Information Management Release of Information at 425.690.3406. Medical records will be mailed or faxed to the requester as appropriate.
